Blue Cross - SmartSense 500 w/Gen RX
Reliable, basic protection with some of our lowest monthly rates. Choose the annual

that works for you. Immediate benefits for the first three visits to a doctor. Your

Aetna - MC 2500
This PPO Plan features
a $30 office visit copay for your primary care physician, $40 copay for a specialist,
$2500 deductible then coinsurance at 30% up to $7000 maximum out of pocket. The
health plan has a $5,000,000 life time maximum.

This plan covers the first two office visits with a $40 copay, then is subject to
the $2900 deductible Generic perscription drugs are covered with a $10 copay, and
hospital coinsurance is 40% after
the deductible. Life time maximum is $3,000,000

Health Net - Simple Value 30 w/Gen Rx
A zero deductible plan with a $30 office visit
co-payment and 30% coinsurance at the hopsital and for lab work. The 30%
coinsurance caps out at $7,000 annual maximum per member. Generic perscriptions
are covered with a $10 copay. And the lifetime maximum is benefit is $5,000,000.
