Small Group Health Insurance is available to businesses with two to fifty employees.

a sole proprietor, partnership, LLC (Limited Liability Company), or a corporation,

Business Check-Up
Many of the basic requirements are the same for different businesses. Your business
must have been established for at least 45 days, or formed in the previous calendar
quarter. You will need an EIN (Employer Identification Number) from the
IRS,and your state business
entity paperwork on file with the
Secretary of State's office
if applicable.

Sole Proprietorship
If you are a sole proprietor of a business, you will need a second individual on
your payroll to qualify for guaranteed-issue group health insurance. The form required
for proof of employment is DE-6 from the
Employment Development Department.
Partnership
If you have not formed your business yet, a partnership has several advantages over
other business types when acquiring group health insurance. You will need a business
license from your local municipality and an EIN.
Limited Liability Company - LLC
LLCs are established through the
California Secretary of State's
office.An LLC is more expensive to create than a
partnership
or
sole proprietorship
and must pay an annual $800 tax to the state of California every year. This is a
large expense for many of our small business clients.
Corporations
There are two categories of corporations: the S Corporation and the C Corporation.
Most small businesses prefer to avoid the double taxation of the C Corporation format
and instead take advantage of the S Corporation's tax liability pass-through. Both
types of corporations must pay a yearly $800 fee to the office of the
Secretary of States office.
Depending on the business form, you will need a K-1 or 1120 tax form from the previous
tax year to establish business ownership. Also, any employees applying for coverage
will need to appear on the last two payroll cycles.
Workers' Compensation
Businesses without employees are exempt from having Workers' Compensation in place.
However, businesses with employees on the group health coverage are required to
show Workers' Compensation coverage in place as well. Depending on your industry,
there are several different good carriers available, and rates start at a minimum
annual premium of $500.
